"Painkiller Overdose" is pretty underrated and actually improves on some of the things from the original game, but it's also really rough around the edges and has incredibly uneven difficulty, so you need patience to really see the better parts of it. The biggest problem with the original Painkiller was the fact that you could play pretty casually and just spam rockets and switch to the shotgun up close. This game's weapons are way more balanced in that you will be using every weapon on every level if you even want a chance to survive. The shotgun is still powerful but you have to be much closer to deal lethal damage. The rocket launcher weapon is totally nerfed and doesn't even kill enemies in a single hit. This means you will likely use it but it's not going to overpower the other weapons. you also get a homing dagger, a remote explosive and a cool voodoo skull that shoots a laser and deals hard damage up close. Most people who play this game won't see how this is an improvement over the original, but if you really look at the balancing and the fact that you can't just play however you want, it makes more logical sense to force the player to think on their toes and switch weapons to survive. I'm also really impressed by the level concepts in this game, even though the level designs could use some work. The game starts with a hellish version of Ancient Greece. There's also a movie studio, a farmhouse with cows with their stomachs split open and violent birdhouses that launch chickens, an airship, a fantasy-themed level, a civil war reenactment, and a city scene where zombie police are fighting zombie thugs. They are some of the most creative level concepts I've seen in an FPS, and there are a ton of enemies. Most levels introduce 2-3 new enemies on top of ones that existed in previous levels, and they all have their own mechanics, like a cutout of a wolfman whose weakpoint is the beating heart attached to the backside of him, or some crazy pilot women in latex who launch themselves at the player with an unavoidable drill attack. You really need to think carefully about how to proceed on a lot of these levels, especially on Trauma difficulty (The highest setting). It's impossible to beat Trauma difficulty without the tarot cards the game gives you because there are certain areas where you will be locked in a small room and enemies will instakill you. Certain cards will give you invicincibility or let you clear rooms just by walking around, so knowing to use them at certain parts of the stage is critical. I managed to beat it on trauma and it was pretty painful but if you know the game mechanics the game gets a lot easier. You have to learn the enemy behavior patterns to proceed, and because there are so many enemies you can expect to take 20+ hours to finish this on the higher difficulty settings. With all of that said, the character you play as, Samael, is pretty obnoxious. and this was a game made as a fan game that got funded by the company, so you can expect the game to just have problems. Most specifically the hit detection enemies that deal splash damage. This will be infuriating in the beginning of the game because you will get instakilled before even being accustomed to the controls. An issue with all of the "Painkiller" games is that on Trauma difficulty you will not get health refills, so the only way to win is to play the first few sections perfectly and reload you save if you take damage. The load times in this game are pretty horrible, expect to wait 25 seconds to get thrown back into the action, assuming the game doesn't freeze completely and lock your computer which happened to me several times. Despite these flaws this might actually be my favorite Painkiller game, if only because it really makes you master the weapons and know your enemies to survive.

Tl;dr: I'm conflicted on this game. It's a good painkiller game with annoyances, tedium and crashes. Tastes like chicken. The original painkiller was a lot of fun with a few annoyances. This game is a lot of annoyances and a few pieces of fun. The story intro is pretty good, they present the character well, great VA making the protagonist feel like a strong revengful half demon. It plays and feels much like the original, with immersive and creative environments, creative and unique enemies, card challenges, souls and a new set of weapons that borrow a lot from the original weapons. But for some reason, all weapons feel less satisfying and they added tedium everywhere. I always run out of ammo. Always. Sometimes on most of the guns. This is good, because it forces me to utilize most of my guns. It's bad because I have to spend more time searching for ammo boxes that blend into the environment. I'm too often forced to mimic Judge Dredd when he was pinned down by the other judges in the 2012 movie. Hiding somewhere, then checking each weapon. Shotgun: empty, rockets: empty, minigun: empty, demon head: empty, arrows: empty, cluster bombs: empty, heart grenades: 13 left. Guess I'm using heart grenades then. Enemies have incredibly narrow/small hitboxes, easy to miss. I've seen shots go through their bodies without hitting, because the hitboxes are too small or poorly placed. I'm not an expert at aiming, but I'm not THAT bad at aiming. Spawn locations are really obvious in this game. The last game at least tried to give the illusion of enemies flowing in from somewhere else. This game, they mostly spawn in rapid succession in open space. Secret hunting is no fun. There are so many visually wide open spaces that are blocked off by invisible walls, that I mostly stopped even trying. The original had you climbing up broken lightpoles and jumping across tiny poles sticking out of a burning roof, i.e. places you'd never think to even look to get that holy item. It was hard to find an invisible wall. This game feels a lot more linear, because everything is blocked off with invisible walls, bar a few non-obvious exceptions. They still let you jump off ledges. Not all ledges, but most. The inconsistency makes it hard to know what to trust. Exploration is further hindered by areas that keep throwing an endless barrage of mortars, meteors or other damaging elements at you. You're not allowed to stay to look around. Move on, or die. Tentacles keep spawning behind you in tight spaces in the swamp level. Hitscanners keep hitting you from the first moment they spawn. Melee attacks don't hit when the animations shows a hit, but on the first frame the attack animation begins. I died too many times because I didn't understand what hit me, when it was the melee attacks being timed wrongly. Gas grenades are weird. If the grenade hits you, you take damage over time. The green cloud they emit? Not sure if that damages you, but the grenade object certainly does. I think they forgot to animate the scorpion enemy in the desert level. You know the desert level? The level with serious sam vibes, and the wolves/coyotes/whatever they are that howl. They howl constantly. It's probably a good audio atmospheric element, if they didn't play it on repeat nonstop without delay at semi-loud volume. I found a few softlocks on this level. Card challenge hunting is made harder in many ways. E.g. not all levels are backtrackable. The original game did an excellent job of making sure all levels are backtrackable (though I got lost on the military base). This game is hit or miss from map to map. I like that cards are more expensive, but dislike that the margin for error on challenges is unreasonably tight in some cases. One level wants you to gather 160 souls to get a card. There are 162 souls on the map, and most enemies fly off into the distance when shot because of physics. Even if you manage to gather the souls, the last fight is in a tight room. If you're in the center of the room at the last kill, you are ambushed by the end-level orb and robbed of the ability to gather those last few souls. Another level wants you to gather 400 gold. The level has 412 gold. Gold despawns. It's a nightmare to prevent 12 gold from despawning while also finding enough. Another level introduces a new gun, but also has a "only use the base weapon" card challenge. Can't use the new gun if you want the card. There was no reason for this card challenge on a level that introduces a new gun. Oh, and they changed the order of the guns. I keep rocketing myself because the rocket launcher is on '3' now. Also, rockets are really weak (except the ones that damage yourself, of course). Enemy pathing is sometimes really bad, they get stuck chasing directly towards you. I was sometimes jumping around looking for the way forwards.. but everything was locked. I had to check the gps arrow to find enemies stuck behind a railing. I got to the viking level, and then the game started crashing mid level. If there wasn't so much tedium and sources of annoyance, I'd keep playing. I do recommend it for its good parts. Just be mindful that the game might annoy you.

Slightly worse than Black Edition. Pros: + new weapons + game encourage you to play different weapon because you are constantly low on ammo + enemies variety Cons: - there are many balance issues like tentacles in Marsh level. They can reduce your HP by 60-70 points easily. Yeah, some enemies are too strong and have too much HP, others don't and seems to be too weak. - game crashes when you want to change your Cards. I think you need to put away that one card which give you +5 HP and then put it again as the last one you are gonna put. Weird. - battle music is kinda weak - 400 gold requirement for getting card in Japanese Massacre is really frustrating. Gold coins can disappear quickly so good luck It is good idea to give it a try

Believe it or not, my dad used to let me play this when i was like 13-14 years old xD the only thing i was scared of was that head weapon since whenever you pull it out the character inspects it and i was terrified of it, shit was so scary to me i was forced to use only that shotgun weapon but oh well my "childhood" game, anyway the game itself is awesome and im glad i can buy it on steam to replay this gem like i used to before <3

Very cool horror shoot-em-up style game. I will say sometimes when it loads you back in, it'll bring you back in at the beginning of the chapter. And sometimes the game does crash even though I have the recommended specs.
