This is the only game of Painkiller series which I have in my library. I was never a fan of this type of FPS and the only games of this genre I played were Doom and Doom Eternal. But I still decided to try Painkiller Overdose in 2026 because I remember Painkiller games from 2000s and I even saw their DVDs back then. And I was very surprised that this game actually runs well on my modern system. However, it's hard to actually enjoy playing it and the reason is not because it is very old. This game was dated already by the time of its release. Shooting enemies can be sometimes fun but the game just tried to hard to be a parody and rather looks more like a bad mash-up. The balance is also just bad. But at least there are cheats codes.

Painkiller Overdose is a mixed bag. For standalone expansion over Painkiller Black, I think it does a good job in providing engaging maps, new weapons, enemies, and a good amount of variety. Some of the levels are phenomenal, Japanese Massacre was incredible, Village of the Damned feels like an RE4 map, and Haunted Valley is just my favorite level I have played so far in a Painkiller game. The enemies are great, and the map layout, besides being the most beautiful map ever, is also well balanced and fun. What can I say? I love some close-quarters combat. All weapons are "original", nothing is rehashed from Black. Now, why the quotes? Well, that's because most of the weapons are just retextures. For example, I know the radiation gun is a reskin of Stake Launcher with Flamethrower as alt fire. I also know the shotgun is the exact same from Black, but with mud instead of ice. I do commend the team tho, the real new weapons like Eggs and Evil Eyes are major fun to use, and the retextures are good and fit really well. I even think the Cannon-Machine gun from Overdose is better than Black, shoots faster, no BS (tho the missiles go through enemies sometimes). This "remix" happens with enemies too, but it is also very well made and has good quality. Now, for the bad, the most obvious issue is the ♥♥♥♥♥♥♥ gigantic loading times. Bro, what the ♥♥♥♥? Almost 2 minutes to load a map on a 2007 game on a 7300MB/s NVMe is just unacceptable. I tried a few workarounds that made the loading times a little smaller, but ye, sucks anyway. Also, the game CRASHES constantly. I had 5 crashes in the Loony Park map, which is a remix of a map from Black that ran just fine. On a minor note, Belial is a little annoying. Don't get me wrong, the catchphrases are fun for the first 10 times, at the 150th, you really just want him to shut the flying ♥♥♥♥ up. While I understand why Painkiller Overdose reviews sit at a mixed rating, and while I do think it is inferior to Black and BooH, my favorite moments from Painkiller happened here. (And yes, I am talking again about Haunted Valley, what a great level). Now, I've read online that this is the only expansion with original assets, since the others are rehashed maps and remixed stuff from Painkiller Black and Overdose, I am stopping with the classics around here, hehe.

Buy if you want, this is not really as much fun as Painkiller Black Edition. Replaces Daniel Carner with Belial, both play the same, except Belial has 5 more HP which is pretty much nothing and uses different set of weapons. Unfortunately Overdose is more stingy with ammunition and have less of Tarot Card options (bunch of them are outright obsolete and useless, such as Ascendance (+5 HP) and Hellish Armor (20 untagged armor which causes it to be just a number as only Bronze, Silver and Gold armor are actual armor stats). New set of enemies copy more from Battle Out of Hell, meaning there are enemies who'se attacks are near impossible to avoid.

Pretty much just the first game but with slightly better level design.

so yeah basically you start the game right after the ending of the first game where jeremy painkiller has finally collected every pharmaceutical drug in the world and at the beginning of this game he overdoses and dies immediately so im not sure why they made a whole game when it ends right at the beginning but overall pretty good just dont buy it instead watch markiplier play it and also i recommend playing king of bowling for the playstation 1 instead
